Hi,
I have listed down a few steps you can follow:
1. Start simple and easy by trying to describe things around you in English, either in your head or with someone present.
2. Try your best to not translate the grammar part. For the vocabulary however; whenever you are stuck, consult a dictionary and make a few sentences with the new words to enforce them into your memory.
3. Gradually develop steps 1 and 2 to advance your skills.
4. Don't be afraid of making mistakes. It's the only way you will learn and improve.
